What You’ll Be Doing:

As a Family Connector, you will play a vital role in supporting families who have loved ones who may need support from the health service or local authority. They help families navigate key processes, such as needs assessments and safeguarding, while fostering peer connections and empowering them with knowledge and confidence to advocate effectively.

Key Responsibilities:

  • Provide clear information to families about processes such as needs assessments, safeguarding, and care planning.
  • Help families understand their rights and entitlements within health and social care systems.
  • Build supportive relationships between families, encouraging peer support and shared learning.
  • Signpost families to relevant local services, advocacy groups, and resources.
  • Offer practical guidance on how to effectively communicate with professionals involved in their loved one’s care.
  • Attend relevant training and team meetings to stay informed about policy updates and best practices.
  • Maintain confidentiality and always uphold safeguarding principles.
