The Department.
People First’s Cumberland Carers service delivers carer support services across Cumberland, working with unpaid adult carers to identify needs, provide information, complete assessments and develop personalised support plans. The service works in close partnership with statutory services, health providers and voluntary organisations to ensure carers receive timely, appropriate and holistic support.
Purpose of the Role
The Adult Carer Support Worker works directly with unpaid adult carers to identify needs, complete carer assessments and provide tailored information, advice and support. The role supports carers to sustain their caring role, improve wellbeing and access practical, emotional and community-based support.
Key Responsibilities
Identification and engagement of carers
- Proactively identify and engage unpaid adult carers through outreach, referrals and partnership working.
- Engage carers in a strengths-based, trauma-informed, person-centred way enabling them to express their needs, outcomes and priorities.
- Promote awareness of carer’ rights and the availability of support under the Care Act 2014.
Carer assessments and support planning
- Complete carer assessments and co-produce appropriate, person-centred support plans via face-to-face meetings, telephone and digital platforms.
- Review support plans as required and respond to changing needs.
Information, advice and signposting
- Provide accurate, up-to-date information and advice on caring, wellbeing, services and community resources.
- Signpost carers to appropriate statutory, voluntary and community services.
- Provide information and guidance on benefits and refer to specialist support where appropriate.
Preventative and well-being focused support
- Support carers to access respite, breaks, peer support, training and wellbeing activities
- Support carers to plan for emergency and contingency situations to reduce risk of crisis.
- Promote independence, resilience and sustainability of caring roles.
- Facilitate the development of networks of Carers across Cumberland, supporting them to become self-supporting and linked to other organisations that can provide on-going support where required.
Partnership and multi-agency working
- Work collaboratively with statutory, voluntary and community organisations to support carers.
- Maintain effective professional relationships with health, social care and community partners.
- Support the development of links and partnership with health services in Cumbria both in acute and community settings.
- Promote awareness of carers’ needs and the People First carers service across agencies.
- Maintain and develop existing carers support services run in partnership with health care staff including the Carers & Discharge Services in West Cumberland Hospital
- Support and deliver Carers Champion training to employers within Cumbria.
Recording, monitoring and reporting
- Maintain accurate and timely records on People First and local authority database systems (training provided)
- Record assessments, support plans, outcomes and case notes in line with guidance and data protection requirements.
- Provide monitoring information and statistics as required for service delivery and funding.
Quality, compliance and organisational responsibilities
- Work in line with People First policies and procedures, including safeguarding, confidentiality, equality and health & safety.
- Contribute to quality assurance, service improvement and externally audited standards.
- Participation in supervision, team meetings, training and development activities.
- Contribute to a supportive, inclusive and collaborative working environment.
Skills, Knowledge, and Experience
- Experience of working directly with people in a supportive, helping or community-based role, demonstrating strong people skills, empathetic, non-judgemental and able to build rapport with a wide range of people.
- Ability to listen, engage and communicate confidently with a wide range of people including customers and professionals via various forms of communication including face to face, over the phone, via apps and in writing.
- A person centred approach that recognises individual strengths, choices and outcomes.
- Ability to work independently, manage a varied caseload and work flexibly across a geographical area.
- Ability to complete assessments and develop outcome-focused support plans.
- Good IT skills, including use of databases and digital communication tools.
- Numeracy skills.
- Positive attitude and ability to adapt to changes in a fast-paced working environment.
- Creative, flexible, and enthusiastic approach
- Ability to work flexibly, including occasional evenings and events.
Qualifications
- Good all-round level of Education
Other Duties

Due to the developing nature of our organisation, we require staff to remain highly flexible in their approach, and work to meet the changing needs of the projects.
- Post is subject to satisfactory DBS Clearance.
- A full driving licence and access to a vehicle with business-use insurance due to travel across urban and rural areas.
