Hospital discharge: tell us your thoughts

Did you know there are 4 different hospital discharge pathways in the NHS?

These run from 0 to 3 and relate to the level of care you need when you are discharged.

What are the different pathways?

Pathway 0 – if you are discharged under Pathway 0, this means you do not need any further support from a health or social care service.

Pathway 1 – Pathway 1 means you are being discharged with plans for a care needs assessment.

Pathway 2 – This pathway relates to a need for rehabilitation in a bedded setting.

Pathway 3 – Pathway 3 means you have experienced a life changing health incident and are will not be able to return home once you are discharged.

It is the NHS’s role to make sure that the most appropriate discharge pathway is chosen, so that you can receive the care you need, in the most suitable setting.

The Healthwatch Cumberland team are looking at what improvements and changes you would like to see in Pathway 1 discharges from hospital. If you have recently been discharged from hospital with pans for care at home, then you were likely discharged through Pathway 1.

We’re working with Cumberland Council and North Cumbria Integrated Care to find out local people’s thoughts.

Engagement Manager Chloe has been joining ambulance crews to speak to some local people on their journeys home from hospital. Did you get to tell Chloe your thoughts?

"I spent some time with the Central Ambulance Service and their Discharge to Assess patients, to get some insight around the logistical process and seeing how the transport works as part of our project. The project is independent but well supported by both the local authority and NCIC as they are keen to hear the experiences of this particular cohort to help improve the pathway and service delivery."
"We are asking those who are discharged from hospital on a pathway one, what they felt worked well and what could have been better. I had the pleasure of spending time with the drivers and was able to observe first hand the care and compassion that they have for the patients they support to get home. This is ongoing and I hope to spend more time with the team to get to know more. If you or a loved one has been a discharge to assess patient, we would love to know your thoughts in our survey!"
